After 81 Years, Jehovah's Witnesses Adjust Blood Transfusion Policy

Jehovah’s Witnesses have announced the adjustment to allow members to decide based on personal conscience whether to accept or donate the four main components of blood, a shift that applies to all congregations worldwide, including the organization’s membership across Nigeria’s 36 states and the FCT.

The Governing Body disclosed the change in a statement issued by its World Headquarters in Warwick, New York, and published on its website on Friday. For Witnesses in Nigeria and across Africa, where the faith has a significant presence, the shift raises immediate questions about how local congregations will handle medical decisions that were previously non-negotiable.

According to the statement, “the decision to accept red cells, white cells, plasma, or platelets from another person’s blood is a matter of personal conscience.” That covers every one of the four primary blood components that hospitals routinely use in trauma cases, cancer treatment, and major surgery.

Likewise, each witness must now decide whether or not to donate blood for the clearly expressed purpose of providing components or fractions for use by another person. Both the receiving and the donating sides of the transaction fall within what the organization is now framing as individual religious freedom.

The long-standing refusal of whole-blood transfusions on religious grounds, a core teaching based on the command to “abstain from blood” (Acts 15:20), is unchanged. That line, the organization made clear, is not being moved by this adjustment.

International spokesman Paul Gillies, speaking from the World Headquarters, said blood retains its sacred status within the faith. “We recognize that medical decisions involving blood components are a matter of conscience between each witness and their God, Jehovah,” Gillies said.

The organization said the latest adjustment “follows earlier updates regarding blood fractions and the use of one’s own blood during medical procedures,” adding that “since the Bible does not comment on such decisions, they are a matter of individual conscience. That same reasoning has now been extended to the four main components of blood.”

Friday’s announcement is the second significant revision this year. In March, the Governing Body updated its medical guidance to allow members to decide whether to undergo autologous blood collection, storage, and transfusion as part of their care, while its longstanding prohibition on allogeneic blood transfusion remained unchanged.

Gillies also confirmed that local congregations will not insert themselves into members’ treatment decisions. “Congregations do not involve themselves in personal decisions that rest with the individual conscience,” he said. “We will continue to respect and support the personal decisions of our fellow believers.”

On medical treatment, the organization said its members seek professional healthcare while maintaining their religious convictions, adding that “Jehovah’s Witnesses love life and want the best available medical care” and “do not practice faith healing, and they seek out doctors who can provide effective treatment in accord with their wishes, values, and beliefs.”

Gillies, mid-statement, also acknowledged the role of physicians who navigate these decisions with members. “We are grateful, too, for the many skilled and compassionate physicians who provide quality medical care that respects our personal wishes regarding blood,” he said.

For doctors treating witnesses in Nigerian public hospitals, where blood supply is already strained and shortages are frequently reported, the change introduces a new variable. A patient who previously refused all component transfusions may now consent, but the decision rests entirely with the individual and cannot be assumed without direct confirmation from the patient at the time of treatment.

The Governing Body made the announcement on Friday, September 18, 2026, from Warwick, New York. No implementation timeline was attached to the statement, and the organization did not indicate whether written guidance would be issued to hospitals or individual members.
